It's time to get ready for Summer 2014! - Please read everything on this page!

The 2014 Camp Kalsman Family Guide is provided to give you an overview of much of what you need to know and do in advance of the summer. Please read it!

Please note that the Camper Information Form , the Travel Form, and the Health History & Exam Form  are all due by April 1st, 2014. See below for instructions.

You should also use the "Ready-for-Camp Checklist" below as a guide for identifying all of the things you need to get done before camp starts.

Important:

If your child will be taking any medications and/or supplements in PILL FORM (whether prescription or over-the-counter) on a DAILY BASIS while at camp this summer, you must read carefully and comply with our "Medications at Camp" policy. Failure to comply will result in a $25 per medication re-packaging fee.

 

 

FORMS TO BE COMPLETED
BY APRIL 1, 2014
Camper Information Form
Must be completed for each individual child attending: Click the link below to sign in to your online camp account to fill out the Camper Information Form. This is where you will make any bunk requests and let us know about your child's needs, likes, personality and more.
 
Health History & Exam Form (5 pages)
Must be completed for each individual child attending: Click the link below and print all 5 pages of the Health Form and complete all 5 pages by hand.  This 5-page form includes the Exam and Immunization pages to be completed and signed by your child's primary care physician. Please download and read our Vaccine Policy to be sure your child complies.  If you have not already scheduled your child's camp physical, you should do so right away!

Once you have all 5 pages of the Health History and Exam Form completed and signed, please return the form to our registrar, Heidi Love, by April 1st. Preferably by scanning and emailing it to her at hlove@urj.org, or by sending it by mail to URJ Camp Kalsman, 3805 108th Ave. NE, Bellevue, WA 98004.

 
Travel Form
Must be completed once for each individual child attending camp, whether they are arriving by air or by car.
 
For campers arriving & departing by air, please schedule your flights to arrive at Seattle-Tacoma Airport between 9:30am and 11:30am. Please schedule departing flights between 11:30am and 2:30pm. NO departing flights should be scheduled before 11:30am.
