It's time to get ready for Summer 2015! - Please read everything on this page!

All Camper Health & Info Forms are due by April 1st, 2015. See left-hand column below for instructions on completing these important forms.

The Camp Kalsman Family Manual is provided to give you an overview of what you need to know and do in advance of the summer. If you are new to Camp Kalsman, please read our Special Guide for New Campers! You should also use the "Ready-for-Camp Checklist" below as a guide for identifying all of the things you need to get done before camp starts. Medications At Camp:

If your child will be taking any medications and/or supplements in PILL FORM (whether prescription or over-the-counter) on a DAILY BASIS while at camp this summer, you must read carefully and comply with our Medications at Camp policy. Failure to comply will result in a $30 per medication re-packaging fee.

Vaccinations:

We require full vaccination of our campers. Please click here for the list of required immunizations and make sure your child is up-to-date according to our requirements.

CAMPER HEALTH & INFO FORMS TO BE COMPLETED BY APRIL 1, 2015
Please complete the 6 forms below via the Forms & Documents section of your online CampInTouch account
1-Health History Form (complete online)
Click here to sign in to your CampInTouch account, then click the Forms & Documents link. The Health History Form must be completed online. This form is where you will provide your child’s health history, including their complete immunization history and medication needs. We require full immunization of our campers and a complete immunization record is required for attendance. Please click here for the list of required immunizations from our Medical Director, Dr. Loren. If your child will be taking medication while at camp (including over-the-counter meds or supplements and/or vitamins), please click here to read our Medications at Camp policy very carefully to be sure you follow proper camp procedure and to avoid our $30 fee.
2-Medical Examination Form (print out)

Click here to download and print the Medical Examination Form. This form must be completed and signed by your child's physician based on a recent exam (within 12 months of the date your child will be arriving at camp). Once completed and signed, please scan and upload this form to your CampInTouch account in the Forms & Documents section. If you don't have a scanner, please mail it to URJ Camp Kalsman, 3805 108th Ave. NE, Bellevue, WA 98004.

3-Insurance Card & Authorization (print out)

Click here to download and print the Insurance Card & Authorization Form. This form must include a copy of the front and back of your child's health insurance card. The bottom of this form must be signed by both the parent and the camper. Once completed, please scan and upload to your CampInTouch account in the Forms & Documents section. If you don't have a scanner, please mail to URJ Camp Kalsman, 3805 108th Ave NE, Ste. 100, Bellevue, WA 98004.

4-Camper Information Form (complete online)

Click here to sign in to your CampInTouch account, then click the Forms & Documents link. The Camper Information Form must be completed online. This form is where you will make any bunk requests and let us know about your child's needs, likes, personality, and more.

5-Travel Form (complete online)

Click here to sign in to your CampInTouch account, then click the Forms & Documents link. The Travel Form must be completed online and must be completed for each child attending camp whether they are arriving by car or air. This is also where you will tell us the names of those who are authorized to pick your child up from camp.

For campers arriving & departing by air, please schedule your flights to arrive at Seattle-Tacoma Airport between 9:30am and 11:30am. Please schedule departing flights between 11:30am and 2:30pm. NO departing flights should be scheduled before 11:30am.

6-Anti-Bullying Pledge Form (print out)

Click here to download and print the Anti-Bullying Pledge. This 2 page form must be read and signed by both the camper and the parent for each child attending camp. Please read and discuss page 1 with your child and keep page 1 for your records. Both parent & child must sign page 2. Once signed, please scan and upload page 2 as a PDF to your CampInTouch account in the Forms & Documents section. If you don't have a scanner, please mail to URJ Camp Kalsman, 3805 108th Ave. NE, Bellevue, WA 98004.
